Oldham County Community Health Survey

Your opinion is important! If you are a resident of Oldham County and at least 18 years old, please take a few minutes to complete our survey. You can enter your phone number or email address at the end of the survey for a chance to win one of many $50 gift certificates. The deadline to receive surveys is April 16th.

The purpose of this survey is to get your opinion about community health issues in Oldham County. The Coalition for a Healthy Oldham County will use the results of this survey and other information to identify the most pressing problems which can be addressed through community action.
